After reviewing your medical history, your doctor or the anesthesiologist may recommend specific pre-admission tests. If this is necessary, your doctor will send you to a convenient testing site; his or her office staff will help you make these arrangements. Lab work must be completed at least 24 hours prior to surgery, unless otherwise instructed by your doctor.
Once you have received a UPMC Hamot Surgery Center information folder from your doctor’s office, please complete the following forms and return them to us in the addressed, postage-paid envelope included in the folder:
- The green or pink-colored Patient Questionnaire
- Patient medication sheet
- Signed Consent to Operation form – (if you haven’t already completed this form with your doctor, it will need to be done when you arrive at the Surgery Center)
- The blue-colored Patient Disclosure Form
In order to prevent a delay in your scheduled time for surgery, it is important that we receive all of this information as soon as possible.
Arrange for a reliable adult (18 years or older) to listen to your discharge instructions with the nurse, and give you a ride home. The person who will be driving you home and listening to instructions after your procedure needs to be available to speak to our staff by telephone prior to your procedure if they do not come into the building with you. If you do not have a ride, it will be necessary to cancel your surgery. You are also advised to arrange for a responsible person to stay at home with you for 24 hours post-operatively. If you are only receiving a local anesthetic, you may consult with your doctor regarding the need for a ride home and assistance at home for the first 24 hours after your surgery.
Two or three days before your scheduled procedure, we will call you at the number you provided on the green patient questionnaire. You can expect a call between 8:00 a.m. and 4:00 p.m. If you know you will not be available during these times, please feel free to call us Monday through Friday between 8:00 a.m. and 4:00 p.m. at (814) 453-3900, extension 229 or 374. During the phone call we will review your preparation for surgery and answer any questions you may have.
