PREPARING FOR
YOUR SURGERY

STEP 1

STEP 2

Arrange any pre-admission tests your doctor or the anesthesiologist may recommend.

  • If this is necessary, your doctor will send you to a convenient testing site.

  • Lab work must be completed at least 24 hours prior to surgery, unless otherwise instructed by your doctor.

STEP 3

Fill out the following forms from the UPMC Hamot Surgery Center folder you receive from your doctor's office:

  • The green or pink-colored Patient Questionnaire

  • Patient medication sheet

  • Signed Consent to Operation form – (if you haven’t already completed this form with your doctor, it will need to be done when you arrive at the Surgery Center)

  • The blue-colored Patient Disclosure Form

STEP 4

Please return these completed forms to us in the addressed, postage-paid envelope included in your folder.

STEP 5

Arrange for a reliable adult (18 years or older) to listen to your discharge instructions & give you a ride home.

  • This person must be available to speak to our staff by telephone prior to your procedure if they do not come into the building with you when you arrive.

  • We recommend that you arrange for a responsible person to also stay at home with you for 24 hours post-operatively.

  • If you do not have a ride home, you will have to cancel your procedure.*

    • *If you are only receiving a local anesthetic, you may consult with your doctor regarding the need for a ride home and assistance at home for the first 24 hours after your surgery.​

STEP 6

Expect a call from our office 2 to 3 days before your procedure between 8AM-4PM at the number you provided on the green patient questionnaire.

  • During the phone call we will review your preparation for surgery and answer any questions you may have.
